[ Intention vs Action ]

Does it matter what one's intentions are or only one's actions?
For example, once a friend offered to help clean the mess after tying her shoelaces at my front gate (literally like one minute before leaving the house). Unfortunately i understand that although she pretends to help she doesn't really care, not at all (since i won't possibly ask her to untie her shoelaces and come back wash the dishes...)

And how i should respond has become my dilemma - 
Should i risk our friendship and point out the elephant in the room that come on, obviously you don't want to help!
Or, even after acknowledging her intentions, should i pretend to not being aware of it at all and still thank her for her far-from-perfect acting? And if so, will she acknowledge that i'm just trying to pretend that i'm not aware of her pretending to help?
